Baptismal Certificate: We will need the candidates, Full name (as it appears on your birth certificate), Date of Birth, Date of Baptism, Parent's name, Godparent's name and the requesting parties contact information.

Baptism certificates with NOTATIONS: Have your  Communion and Confirmation information. If possible, please provide the certificates at the time of request. 

Communion Certificate: We will need the candidates Full name (as it appears on your birth certificate), Date of Birth, Date of Communion, Parent's name, Godparent's  name (depending on the date, most candidates don't have communion godparents) and the requesting parties contact information.

Confirmation Certificate: We will need the candidates, Full name (as it appears on your birth certificate), Date of Birth, Date of Baptism, Parent's name, Sponsors name and the requesting parties contact information.

Letter of Good Standing: For the parish to issue a letter of good standing, the requesting party must be a registered parishoner for more than 6 months. 

Please make sure that your account has  up-to-date information. Including: Envelope numbers, postal addresses, contact information etc.  You can do this by signing in to your Parishsoft account or filling out the application again.

Mass Intentions are due the Thursday before the mass at 12pm. 

You have to call or visit the Parish Office and give the persons full name (or their "AKA" name) and the type of intention you would like to give. Examples are, Birthdays, Anniversaries, In Memory, Health,etc. Then which mass time would you like the intention to be named. Please limit your intentions to at most 3 people in each mass. 

Mass Intentions have a minimum donation of $10.

There are applications in the lobby of the main church or you can come into the Parish Office to register.
